Healthcare, "unlimited PTO", some great fellow consultants

Low transparency for rates/margins, poor-to-zero follow through for redeployment, general sloppiness and inaccuracy in any comms (emails go to wrong recipients, SOWs/Job descriptions inaccurate and need to be fact-checked, generally unaware or naive re: HR and workplace rights, referral process in a nightmare, I would never refer a colleague after getting burned/embarrassed several times by the recruiters' treatment of the referred candidate. Poor track record on D&I unless you count white lady as D&I
