The company needs to find its consistent management rhythm - empower people more towards responsibility, give them enough freedom, push them into the deep end, but also MAKE them see to the consequences of not delivering what should have been delivered. A mistake is not a problem, but laziness and lack of work responsibility sometimes create blind gaps and problems.
