Senior Leadership. Every issue within the organization can be traced back to the failures thay come from the higher echelons. Be it implementing a new shiny system that doesn't align with others, or absolutely half baked ideas being launched and forced into implementation even if it just creates chaos and confusion. This is systemic across all orgs and is due to the "ego lead" mentality of most senior leaders.
Unless you are a sycophant or apart of the inner circle, promotions just don't happen. Most people being brought in are close to other leaders even if they lack relevant experience or any leadership acumen. This is seriously hurting retention. Most people don't make it more than a couple of years, and the only ones who stay have their options near fully vested.
Too many cooks in an over crowded kitchen. There are too many Sr. Directors and VP's within the organization that have been here too long. They interject themselves into different projects and tasks that they have nothing to do with, and use their title to impose their will. Removing any and all ownership for the teams who actually are responsible for it. This creates a bureaucratic road map that is impossible to navigate and nothing gets done because five different people all think they have final say in something and none of them can agree (right back to ego leading).
