- Lack of focus in direction - People work in silos with little to no cooperation happening - Very little common vision in the operation - Overall ineffective or lack of management - No focus on efficiency or process improvement - Management afraid to be managers / make hard decisions - Each department is its own silo and have no understanding of the others
