As part of a true interdisciplinary team you'll be able to closely manage the co-morbid conditions of patients at risk for repeated hospitalizations and at risk for living in long term care facilities. Patients are enrolled into the program and have a complete interdisciplinary team who work together to create personalized plans of care for patients. You will not be rushed and limited to a typical 15 minute visit and care provided is truly comprehensive. Medicare/Medicaid rules increase the paperwork burden, coding, utilization review and being responsible for the safety and well-being of patients 24/7 can be overwhelming at times.
