The company grew very rapidly during the 1990s and 2000s (from about 30 employees to over 900). The services and products that the company provided were high quality, but as the company grew it was not sufficiently successful in creating the necessary structure and procedures needed for ongoing efficiency and effectiveness. Although management was aware of what was needed, and tried a number of approaches to bring it about, the needed changes were elusive and not adequately implemented. Consequently, the company finally reached a size that it could not sustain due to its structural and procedural deficiencies and gradually began to go downhill.
