I applied online. I interviewed at Veterinary Data Services
Interview
I applied for a remote position and received an email asking me to contact their in-house recruiter to schedule an interview.
After sending over my availability to the recruiter I was asked to send personal information so HR could run a background check on me and start the on-boarding process...all before even scheduling my preliminary interview. The recruiter would not schedule the interview until I sent over that information.
I questioned why they needed that information so early on in the process and informed them that I felt VERY uncomfortable sending that information before even meeting with anyone and learning more about the position and the company.
I was told this was their standard procedure - to "confirm all documents provided are yours." I declined to do this and not did schedule an interview.
This is a super weird policy, for remote positions or in-house, that I'm sure would make others uncomfortable also. It caught me very off guard so be aware that they may ask for personal information right off the bat.

Question 1
No questions were asked during the interview scheduling process.
I applied online. The process took 3 days. I interviewed at Veterinary Data Services (Lexington-Fayette, KY) in Mar 2016
Interview
The interview is focused on programming tests such fizzbuzz, implementing searching and reviewing how you would structure a basic application. There are a few standard questions regarding database knowledge and evaluating code snippets for errors. They are not really interested in what you have done previously and will judge you based solely on their tests. You will be expected to either write code or pseudo-code on the spot.
The company appears to be very laid back and when I arrived there was someone sleeping on couch in the lobby. If you like a traditional office this is probably not the place for your, if you want to twenty-something startup culture then you may fit in.

Write an application implement the fizzbuzz programming test.
I applied through a recruiter. The process took 2 days. I interviewed at Veterinary Data Services in Oct 2014
Interview
It was a bit awkward, as the other(s) have said. it was 2 people in a room, in T-shirts. And dogs. And halfway through someone interrupted the interview and asked for help from one of the interviewers.
The questions are more of a critical thinking demonstration, it is not about if you can answer the questions flawlessly, they simply want you to show them you can think critically. They will train you in everything you need to do once you start- they simply want to make sure you can think critically and rationalize your processes.
Of course, you should know some technical things, like how to write a query, how to access Task Manager, access the command prompt, but these are all basic.
