I applied online. I interviewed at Stack Overflow
Interview
Five fully-remote interviews over a 6 week period. Each interview was very difficult, but each interviewer was very professional and took the time to help me through answers when my immediate reaction was "I don't know".
The interview questions were pretty difficult and very in depth, but when you get stuck they tended to give you some ideas to point you in the right direction.
You need to be able to pass a programming test to get a job offer as well. Some of the solutions were easy, some of them were more complicated - but nothing even close to as difficult as what you read the senior developers go through.
Quick chat with the in-house recruiter (30 minutes) then technical interview with Quality Engineering Lead that involved a live coding puzzle, we did the "FizzBuzz" problem and more theoretical problem solving puzzle where you had to answer how you would parse a log file.

I applied online. The process took 4 weeks. I interviewed at Stack Overflow (New York, NY) in May 2015
Interview
The interview process was very painless and transparent. The requirements of the position seemed to be a very different fit than what was described in my interviews. However the company was very respectful and kept communication up.
