Have you ever put an employee on a PIP (performance improvement plan)?

No. I am strongly against PIP's. I feel that maintaining such records is a waste of time unless the law requires this to be done or some internal HR policy. I feel that if we hire correctly up front, you can mitigate these types of issues right off the bat. Aside from this, there are times when people do have a life changing experience where work takes a back seat; As a QA Manager my job is to see that coming and mitigate as well as motivate; last resort, California is an at-will employment state, we could let the employee go.
